I have not found a better Cause and Effect matrix than this one. Complete the C&E Matrix prior to proceeding to the FMEA process at the Analyze phase of any DMAIC project. The Six Sigma Cause and Effect matrix must be used as a high level process mapping right after the SIPOC is completed. Like the SIPOC, the C&E matrix should be completed as a team effort (anyone can complete it on their own but you really need team buy-in if you want this to work).
